How can I find another hospital in the network?
Our networks have many high-quality hospitals and healthcare professionals throughout New Jersey and surrounding areas. You can find in-network hospitals and doctors on the Horizon Doctor & Hospital Finder or through the Horizon Blue app.
How do these changes to the Horizon hospital network affect my benefits?
Nothing about your coverage, benefits or premiums is changing because of this situation.
Can Jefferson Health ask me to pay up front for any services received there before March 1, 2027?
You should not be asked to pay any amount beyond your applicable in-network cost share for any covered services received before March 1, 2027. If you are asked to pay anything additional or have questions, please call us.
Can I use Jefferson Health hospitals and doctors if my plan includes BlueCard® benefits?
If your health plan includes BlueCard coverage, you may still be able to receive covered services from certain participating Jefferson Health hospitals and providers in Pennsylvania if they participate with a Pennsylvania Blue Cross Blue Shield plan.
Because participation can vary by location and plan, we encourage you to verify whether a specific Jefferson Health facility is available under your coverage before receiving services. You can check participating hospitals and providers using the National Doctor and Hospital Finder at provider.bcbs.com.
How does this change to the hospital network impact my OMNIA Health Plan?
If you have an OMNIA Health Plan without applicable BlueCard® benefits, nonemergency care received at affected Jefferson Health hospitals beginning March 1, 2027, will be considered out of network and generally will not be covered.
If your OMNIA Health Plan includes BlueCard benefits, you may still be able to receive covered services from certain participating Jefferson Health hospitals and providers in Pennsylvania if they participate with a Pennsylvania Blue Cross Blue Shield plan. Coverage and costs depend on your plan and the participation status of the specific location. Because participation can vary by location and plan, we encourage you to verify whether a specific Jefferson Health facility is available under your coverage before receiving services. You can check participating hospitals and providers using the National Doctor and Hospital Finder at provider.bcbs.com.
What do I do if I am getting care at a Jefferson Health hospital on and after March 1, 2027?
If Jefferson Health goes out of network on March 1, 2027, you may be eligible to continue receiving care at a Jefferson Health hospital for up to 90 days at the in-network level if they are undergoing treatment for:
A serious or complex condition, such as heart attack, stroke or cancer
Inpatient medical or behavioral healthcare services
A scheduled non-elective surgery or post-operative care
Pregnancy
A terminal illness
If you meet the criteria listed above, you should talk to your doctor about getting care at a different in-network facility. If your doctor wants you to continue getting care at a Jefferson Health hospital, the doctor will need to apply for continuity of care.
Your doctor will need to complete the Request for Continuity of Practitioner Care for Medical Benefits form. The form can be found at HorizonBlue.com/copcmedicalform.
You or your doctor can submit the completed and signed form, along with all supporting documentation, to:
Email: [email protected]
Mail:
Continuity of Care Coordinator, PP-12T
Horizon
PO Box 420
Newark, NJ 07101-0420
If you do not meet the criteria listed above, nonemergency care received at a Jefferson Health hospital on and after March 1, 2027 will be considered out of network and you will pay more out of pocket.
Approval is not automatic. Eligibility and the approved continuation period depend on your circumstances, plan requirements and applicable law.
